Supported living We believe that personal
independence plays a significant role in building self esteem and preserving
one's dignity. Supported Living is a
residential service that enables people to live in their own homes while receiving
individualized support from their assigned direct support professionals. As
part of the program, we work with each individual to help him or her acquire,
retain andlor
improve daily life
skills so that he or she can be as self-sufficient as possible. Those in our program are also encouraged to
pursue personal interests and opportunities within their community such as
volunteering, employment, educational courses and hobbies. |

Nursing Services Making sure our clients stay healthy
is one of our top priorities. We offer nursing services at the oversight
level. This means that our clinical
staff will take all of the necessary measures to ensure the timely scheduling
of medical appointments, ensuring appropriate medical professionals are
contacted and medication refills are ordered, among other duties.
Please note that
our nursing service is only
available for those enrolled in our supported living program and is not
offered as a separate service. |

Supported
Employment Services Precision Community Services LLC, is
committed to helping individuals with a desire to work or volunteer achieve
their goal. Through our Supported Employment program, our trained staff will
assist individuals with finding (and maintaining) competitive and meaningful
employment. Supported Employment services are available for those enrolled in
our Community Based Day Program. |

Community Based Day
Services Most people enjoy being active and
productive throughout the day. Precision Community Services LLC, understands
this so we provide Community Based Day services. With the direct involvement
of the people in our program and their Circle of Support, we design daytime
activities that are meaningful, rewarding and productive. Since community
involvement is an important aspect of our program, many of the activities
will take place in integrated settings with people who may or may not have a
developmental disability. |

Personal Assistance
Services Many families choose to remain
living together despite a member's diagnosis of a developmental disability.
We understand the challenges these families face and can provide support
through our Personal Assistance Program. Through this program, caregivers are
enabled to work, attend school and take care of other needs while our trained
staff remains at home (or in the community) with the loved one. |
